Ilizarov Frame Method

The original circular-frame external fixator method — workhorse of reconstructive limb lengthening for four decades.

How it works

Developed by Gavriil Ilizarov in the Soviet Union, this method uses a circular external fixator of rings and tensioned wires. The fixator remains in place for the full distraction and consolidation phases. While slower and more burdensome for the patient than internal methods, Ilizarov remains the most versatile technique for complex deformity correction, infected non-unions, and bone reconstruction — areas where internal nails alone are insufficient.

Pre-op. Intact bone, hardware in place.
Osteotomy. Controlled break, ~1mm gap initiated.
Distraction. ~1 mm per day gap widens, new bone (callus) forms.
Consolidation. Bone hardens; hardware stays for 12–18 months.
